Materials will be shipped to students by the UMA Bookstore.

A copy of this form will be sent to your local campus bookstore as a convienience for your on-campus students.

Please indicate, for each book ordered, whether used books are acceptable. If students need CD media, or other material that only comes with new texts, please leave the used field unchecked.
